1. Teachers should contact their Site Coordinator with their desired program requests. Requests for programs offered throughout the year may be submitted at any time. Please submit requests for programming appropriate for only your grade level.
    
2. All sites should be sure to read the Price Guidelines. Please be sure to contact the Program Manager with any questions regarding the scheduling fees.
    
3. Site Coordinators should fill out and submit the Live Video Program Schedule Request to our Program Manager in one of 4 ways:
       •  Online
       •  Email as a document attachment
       •  Mail
       •  FAX
   
   
   1. Online Live Video Program Schedule Requests: Complete the online Live Video Program Request form using one of the links below. A confirmation page with your input will be generated, indicating that we received the request. This is not a Program Schedule Confirmation, but may be printed as a record of your submission.

4. We will process all requests as soon as possible. Program Schedule Confirmations for all requests will be provided by FAX to your Site Coordinator. If you do not receive a confirmation FAX within 3 working days after submission, contact the Distance Learning Coordinator at the NSC.

1. If you must cancel your participation in a previously scheduled program, your Site Coordinator should contact the Distance Learning Coordinator at the NSC.
2. Please cancel at least 5 days prior to the program date to allow another class to participate.

The Survey is online. After the telecast, just return to the appropriate Program Description page and click on the Live Video Program Survey link found at both the top and bottom of each Program Description page.
